Salary Information: 

Typical hiring wage $26.52/hr at Step 1 to $32.60 /hr at Step 5 Dependent on Qualifications 

Saline County guarantees your weekly hours of 20, 24, or 40. 

Saline County reimburses mileage at the current IRS rate of 67 cents per mile. 

$10,000.00 Hiring Incentive for 40 hours.  Prorated incentive for 20 or 24 hours.  With a 2-year work commitment.


Job Duties Include:


Provides intermittent skilled nursing care to patients in their place of residence in accordance with agency policy and procedure and State and Federal regulations. Performs direct and indirect nursing care to patients in their homes under the supervision of the physician and/or nurse practitioner.  Performs patient care within the Kansas State Board of Nursing scope of practice and the Medicare/Medicaid regulations for home health skilled nursing.  Performs ongoing assessments during visits, documents nursing interventions and findings, and communicates significant findings to the physician and/or nurse practitioner. Performs patient care within the Kansas State Board of Nursing scope of practice and the Medicare/Medicaid regulations for home health skilled nursing.  Supervises the licensed practical nurse, home health aides and homemaker according to regulations by the Center for Medicare and Medicaid Services.  Coordinates all care from start of care to discharge across all disciplines. Provides education and monitoring for disease processes and follows up with the physician.  Acts as an advocate for the patient and family and the welfare of the patient.  Provides community resources and refers patient to other community agencies when appropriate.  Electronically documents all patient care and information in the medical record according to agency policies and Federal and State laws and regulations. Has knowledge of and compliance with Home Health Agency policies and procedures. Must complete the National Incident Management System training. Maintains certification in CPR and yearly blood-borne pathogen training. Reports for work during public health emergencies as needed. Serves on Boards and committees as needed to promote public health as well as the Home Health Program. Position may require weekend and/or evening care.  May perform other duties as directed.



Requirements


Minimum Qualifications:

  1. Must have a Bachelor’s Degree, Diploma or Associates Degree in nursing.
  2. Must meet licensing requirements by the Kansas State Board of Nursing for Registered Nurses and  hold a current licensure for the State of Kansas.      
  3. Must have good communication skills to work with Staff and public in a congenial manner.  
  4. An ability to maintain a professional demeanor. 
  5. Must have basic knowledge of Excel, Word and other PC software.
  6. A proven ability to plan and organize work.
  7. Must be flexible and be able to adapt to different tasks.
  8. Must have a valid Driver’s License and must provide own transportation.


Physical Requirements:


Determined Work Level: Level 3 – Medium Work: The applicant must attain a 2.5 score or above score on their physical capacity profile. Exerting 20 to 50 pounds of force occasionally, and/or up to 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly. Ability to hear directions and ability to communicate ideas by means of the spoken word when interacting with Staff and public. Must have fine and gross manual dexterity to operate the telephone, calculator, typewriter and computer.
