HIV Medical Case Management

Purpose:

The purpose of the Saline County Ryan White Part B HIV Medical Case Management Program is to provide compassionate Medical Case Management services to HIV-positive individuals in North Central and Northwest Kansas.

Navigating the health care system can be overwhelming.  Medical Case Managers will partner with you to develop a comprehensive care plan that addresses your specific concerns and assures your access to necessary medical care and supportive services.  We understand that the costs of caring for yourself when you live with HIV can be very high.  Case Managers are here to assist you by using the Ryan White Program, and other benefit programs like Medicare and Medicaid, to ensure that you are able to obtain the care and medicine that you need.

The Saline County Health Department hosts an HIV Outreach Clinic every other month that is facilitated by providers from the KU School of Medicine.  We work closely with other medical providers so you will have access to Primary Health Care, Dental Care, Specialty Care, Vision Care, and Behavioral Health Care services too.  If you live outside Saline County, we will help you find providers near you.

We have access to Housing Opportunities for People with AIDS (HOPWA) so we can assist you with your housing needs.  We use short-term rental and utility bill assistance from HOPWA and also our Emergency Fund.  We can assist you with arranging medical transportation and can provide mileage reimbursement to you for your travel to appointments.  Case Managers may also be able to assist with additional financial concerns like medication copays, insurance premiums, bus passes, Food Pantry items and grocery cards.
